You have been banned before on our servers for griefing, have two notes about it, and have a ban on another server for griefing. Since this ban is over a year old I'm going to give you another chance, but know that if you find yourself banned again the ban length will be long. You also have a large alt list, so it appears your account was compromised at one point. To be unbanned, change your password and read the rules then reply here stating that you understand and will follow them.
